Cough in children: From Common cold to Concern
Chronic cough is a common complaint in general paediatrics. Some cases are benign and self-limiting, while others reflect underlying pathology. This presentation argues for a guideline-based algorithm for approaching the chronically coughing child. History, examination and a minimal set of objective tests allow the clinician to phenotype the cough. Cough pointers help identify specific underlying causes; where pointers are present, targeted investigation follows. In unspecific cough, management depends on the quality of the cough — wet or dry. A wet cough calls for timely intervention, whereas watchful waiting may be appropriate for a dry cough. The aim is to give the clinician clear guidance on what can be managed in general paediatric practice and when specialist referral is needed.
