In children, the most common primary headaches are migraine and tension-type headaches. The latter are typically bilateral, pressing in quality and mild to moderate in intensity, and not worsened by physical activity, with no associated nausea or vomiting. Migraines in children can also be bilateral but are usually pulsatile, of moderate to severe intensity, and associated with symptoms such as nausea, vomiting, and photophobia, often worsening with activity. Cluster headaches are rare in this age group and present with severe unilateral pain accompanied by autonomic features like tearing or nasal congestion. Clinicians should always look for red flags suggesting a secondary cause, including progressive worsening, morning headaches with vomiting, neurological deficits, behavioral changes, or abnormal examination findings, such as papilledema. In their presence, further investigations such as neuroimaging is warranted.
